Faith Hope and Charity’s Transition Home opened in May of 2012. The addition includes 2 homes in the Storm Lake community, Independence Home and Adventure Home.
Each home serves 4 individuals with special needs ranging in ages 14 to 21. The Transition Home allowed for the expansion of Faith Hope & Charity services to serve children and young adults who have graduated from the Intermediate Care Facility/Intellectually Disabled level of care.
The services allow individuals to live in a home setting in the community. The individuals served have their own bedrooms and staff providing supported community living services to them on a 24-hour basis.
Those served in the Transition Home attend a normal school schedule and integrate into the community through pre-vocational services and day-habilitation programs.
The Transition Home's focus is to assist individuals in being successful in the home and community. Goals are worked on in areas of money management, medication administration, cooking, personal hygiene, community safety and promote progress toward more independent daily living skills.
Faith Hope and Charity's Transition Homes also assists individuals with special needs with community engagement through attendance at community events and activities on a regular basis.
