Description of the Transition Home
Faith Hope and Charity’s Transitional Services program opened in May of 2012. The addition of transition services includes 2 homes in the community. Each home serves 4 individuals with special needs ranging in ages 14 to 23. The transition services program allowed for the expansion of Faith Hope & Charity services to serve children and young adults who have graduated from the Intermediate Care Facility level of care. The services allow individuals to live in a home setting in the community. The individuals served have their own bedrooms and staff providing supported community living services to them on a 24-hour basis. Transition services allows those served to attend a normal school schedule and allows for further integration into the community through pre-vocational services and day-habilitation programs through Genesis Development. Transition Services focus is to assist individuals in being successful in the home and community. Goals are worked on in areas of money management, medication administration, cooking, personal hygiene, community safety and promote progress toward more independent daily living skills. Faith Hope and Charity Transition Services also assists individuals with special needs with community engagement through attendance at community events and activities on a regular basis.
